Please make sure you print out and get signed the Medical Clearance form.  It looks like the Resume to Play Form.

You will also need your child's 4th quarter Report card(print it as soon as school ends, the district takes them down after a couple of weeks).

Please get a head shot picture of your child, and a copy of their Birth Certificate.
